MM DD YYYY Appointment Time * the length of the appointment will depend on the p[project but this is to help me understand when you are available! 9 am 11 am 1pm other What is your budget? * Shop minimum is currently $40 for very small designs expect to pay $40-60 (can work out a deal if you are looking for multiple) for medium pieces expext $60-$150 depending on detail, for large pieces expect $150+ but I also do not want price to be a barrier so let me know what you are working with and we can figure it out. Payment plans also available :) Sobriety Required * I understand that I cannot receive my tattoo if I am under the influence of alcohol or drugs at the time of my appointment. I understand that I must be sober I will get turned away and my deposit will not be refunded I understand Deposits/Cancelation * I understand that the artist will review my booking and respond with an email to schedule a consultation/appointment within three days. Once a date is agreed upon I will submit a $20 deposit that will not be refunded if I do not show.
